Dear Referee:
When submitting your signed games
cards for payment, please follow the instructions below:
1. Fill out the form below.
2. Attach all signed game cards, in date order, to the form.
3. Mail the form and all signed game
cards to: Crossfire Soccer Club, PO Box 852 Woodstock, Il 60098, Attn: Ref.
Coord.
Both the form and signed game
cards must be received
by the last day of the month for payments to be processed in a timely
manner. Any envelopes received after the last day of the month will be processed
the following month. (Hint: mail your form and signed game cards at least 5
days prior to the deadline.)
All payments will be mailed 10 days after the
conclusion of the season
NOTE: If you are a certified referee, proof
of certification must be on-file with Crossfire Soccer Club. Please send a copy
of certification with the referee payment form and signed game cards. You only need to send a copy of
your certification once. Crossfire Soccer Club will keep a copy of
your certification on-file until certification expiration.
